Worker's Compensation:
If you are covered by worker's compensation insurance, the adjuster or nurse case manager must call our worker's compensation specialist, Cindy Ellis at (636) 229-4249 to schedule your appointment. This will verify that you are covered by worker's compensation on the day of your first appointment and provide us with the WC carrier name, address, phone, contact information (case mgr. or adjuster), name of employer, and claim number. We will keep your employer and/or worker's compensation carrier apprised of your status.
